Congratulations on deciding to be a member of the Melaleuca family. You have made a wise decision.
INDEPENDENT AGREEMENT FORM
The paperwork to become a member of Melaleuca is simple. There are two agreement forms to fill out. Let's look first at the Independent Marketing Executive Agreement. Please print neatly using UPPER CASE letters and stay within the boxes. Use a ball-point pen.
In Part One -- Print your name as you want it to appear on commission & bonus checks that may be sent to you. Then your social security number. There is a space for the spouse's name (write it as you would want it to appear on any checks the company may send you). If you have a fax number you can put that in. Fill in your occupation if you wish.
There is a box that you must check if there is two applicants and you are legally married.
Fill in your address, city, state, etc.
In Part Two -- check which level of participation you plan on working for. Each level is described in the page called "How Can Melaleuca Enhance Your Life?" In summary it says Level of Effort One is that you are not interested in a sideline business and want to be a customer. This is wonderful. Level Two means you see yourself sharing and signing up a few people that will generate enough income to pay for your Melaleuca products each month. Category Three is where you plan to spend enough time to earn around $200-$300 each month. Level Four means you plan on working your business several nights every week and helping at least five of your personal enrollees go Director so you can become a Senior Director and earn several thousand dollars a month. Level Five means you will spend every spare moment on your business and build it into a large company owned and operated solely by yourself. This will take hard work and leadership. Your goal is to become an Executive Director and earn income of $10,000-$20,000 per month or maybe more.
Next check which method of payment you want for your Business Kit. Where it says Electronic Checking you can also put Electronic Savings because payments can be made from a saving's account.
Part Three -- All new members are given a chance to buy one or two packages of products much less than the usual cost. The Value Pack has over 45 products and costs $199. The Career Pack includes everything in the Value Pack plus additional products and sales aids. It costs $299. The company is practically selling these products at cost to help new members get a jump start on their business. Melaleuca is a product centered business. The more product knowledge you have, the more confidence and excitement you have which makes it easier for you to share with belief and persuasive, firsthand experience. This offer is good only for the calendar month you enroll and the two months following. Even if you are not interested in building a business it is a great deal to buy these products at such a savings and convert your home to Melaleuca's products.
Those who have bought these packs testify that it was one of the smartest things they ever did. The company allows two packs per business.
The last thing to do on this form is to write your signature. You must be at least 18 years of age. There is a note under the signatures saying that you can cancel this agreement at anytime for any reason by giving written notice to Melaleuca with your signature, printed name, address and Customer Number. That number is your social security number. To send this in writing is protection for you.
CUSTOMER AGREEMENT FORM
The second form is the Customer Agreement. Put a check in the box for Preferred Customer. You have the choice of either making your monthly purchase a minimum of 29 or 39 points.
Melaleuca has divided their product line into two parts -- Melaleuca products and Vitality for Life products. Most of the products are Melaleuca products such as the Melaleuca oil and the cleaning products. The Vitality for Life products are mainly the nutritional and supplement products such as the Access Bar and Provex CV supplements. If you build an organization and someone orders some Vitality for Life products, you will receive a commission on those products if you have ordered 39 points or more of products that month. Let's say that you forgot to order one month and Melaleuca sent your back up order and you were on file for 39 points. They would send your order and you would receive your commission check. If you were on file for a 29 point back up, then you would receive commission on everything those in your organization ordered from the Melaleuca side, but you would not receive a commission from the Vitality for Life products they bought.
Whichever decision you make you can change it at anytime by sending them a letter requesting a change. The best way would be to resubmit this form and send it to them.
Print name, etc. in Part One. In Part Two fill out how you want to be on file for paying for your order when you call each month. If you are going to use a checking account, write "VOID" on a check. I will send it with these forms so that Melaleuca can use the information on the check to notify your bank and get approval for pre-authorization.
If possible, write in more than one form of payment.
Sometime during each calendar month you call their toll free number at 1-800-282-3000 and give your order to one of their friendly operators. He or she will ask you for your I.D. number. This is your social security number. They then ask how you would like to pay for the order. You tell them how. If you are a brand new member and want only to order from your checking account, it will take a few days until they receive your application form, voided check, and contact your bank for approval. If you want to order by credit card, you can, of course, order at anytime. As your enroller, I will call the company today or first thing tomorrow and enroll you over the phone.
After I do this, you can order with a credit card today if you want to. They have a toll-free number for Marketing Executives to call that is listed at the top of Customer Agreement (1-800-262-0600). If you ever need to write to them, their address is at the bottom of the page. If you are paying for your Business Kit by check, make the $29.00 check out to Melaleuca. In states that have a sales tax, add the tax. Marketing Executives can buy Kits and sell them for $29.00 if they wish when they enroll someone.
Operators are on line in Idaho from 6 a.m. to 10 p.m. Monday through Friday. And Saturday from 7 a.m. to 5 p.m. If it is the last day of the month they stay open until midnight. These times are Mountain Standard Time. They are never open on Sunday. If the last working day of the month is a Sunday, they stay open until midnight on the Saturday before. You can also order any other time by automated phone. Also you can order anytime from their website at www.Melaleuca.com.
Section Three is filled out by the enroller who puts in their name, phone and social security number. Skip Sections 4, 5, and 6 unless this an International Enrollment. I will fill out Section 7 by putting the name of the person who will be directly above you. There are 5 spaces under an enroller and they have the option of putting new enrollees under them if they have the space or under others if they have an organization already. Once someone has been assigned a place in the 5 by 7 marketing plan, they cannot be changed. Every time you make an order, seven households will receive 7% of whatever base points you order. They only know your name, phone number and how many points you ordered. They may never call you to thank you for the check they received, but believe me they are all very grateful.
BACK UP ORDER
In Section 8 you can choose which backup order you want. The company has made several of their own you can choose from, but I recommend you make your own. Check the box Select-Pak Backup Order and fill in at least 29 points or 39 points of products on the reverse side. They will calculate shipping later. Look through the catalog and pick some products you think you would like to have sent if you forget to place an order. There are over a hundred products and as time goes by you will have your favorites. For now, just write in some products and later, after you have tried the products, you can fill this part of the form out again listing your favorite products which will cancel out this list. You can do this at any time. To help you decide, I recommend putting down the Vitality Pak and Vita Bears if you have children. The Vitality Pak is 21 points and the Vita Bears is 5. That makes 26 points already. As you can see, it is very easy to reach a minimum order of 29 points and why most people order more than that each month.
When a person qualifies for the status of Director, the company asks for a commitment of 75 points a month. This is reasonable because at that level you are earning more than the cost of that many products and everyone at that level wants to use that many products each month.
MelaCom and Mela Card
If you like you can sign up for MelaCom, a long-distance savings program that Melaleuca has with MCI. Phone rates change all the time, but many feel that this is a good deal. Of course, if you sign up, you can always change. Or if you don't sign up, you can always do so later.
The company also has a Visa and MasterCard. It is an attractive card with the company's logo on it. Some have even signed up cashiers at stores who they struck up a conversation with. The company has tried to get good rates on the card. An added benefit to both MelaCom and the MelaCard is that you receive a special bonus if someone in your organization signs up and uses these services.
The last thing to do is sign the form. You receive the yellow copy of each agreement. I will send the white copy to Idaho today or tomorrow and keep the pink copy for my records.

When you make your order, you might want to buy a few sales aids if you plan on working the business now. They have literature, audio tapes and videos. Sales aids do not have base points. No one earns money because someone bought a sales aid. Be sure to order just what you think you need. Again, if you are not totally happy with any product you order, you can return it and get your money back.
If you plan on working the business right away, you may want to make your first goal to sign up 8 people so you can become a Director. If you sign up 8 people in any one calendar month you will be recognized in the company's magazine. If you send a picture of yourself, they will publish that also.
